- Enroll in benefits Oct. 28 – Nov. 19, 2017. Annual Enrollment is the one time during the year you can choose your benefits, unless you experience a qualifying life event. When you’re ready, make your benefit elections on EmployeeConnect.
- Beginning Oct. 28, 2017, complete your HRQ to avoid paying more for medical. Take the Health Risk Questionnaire (HRQ) by Dec. 31, 2017, or you’ll pay an extra $100 on your 2018 medical premiums and won’t earn incentives for the other Health Management program activities. Plus, the HRQ is a great way to spot small health problems before they become big ones.

- What will happen to your benefits if you die? Annual Enrollment is a good time to designate who you’d want to receive your payable benefits in the event of your death. Remember to update your beneficiaries on EmployeeConnect or call 1.877.687.7272 Monday through Friday, 8 a.m. to 4 p.m. Central Time. You’ll also want to review and update your beneficiary designations on the Fidelity website (log into 401k.com) or by calling the Service Center at 1.877.765.4015.
