  • IRS Form 1095-C: Employer-Provided Health Insurance Offer and Coverage. This form, which you should have received in early February, reports two different kinds of information to the IRS. First, it shows the monthly premium for the lowest-cost option available to the employee. In most cases, this will be You-Only HSA option coverage. You can verify the amount by comparing it to the 2015 Cost for Medical Coverage in the 2015 Annual Enrollment Guide. Second, the form shows who was covered under your medical plan in 2015. If you have questions about your form, or need a replacement, contact the Rockwell Automation Service Center at 877.687.7272.
  • The deadline to submit your 2015 Flexible Spending Account (FSA) claims is March 31, 2016. Submit your claims by uploading them online via EmployeeConnect > Your Benefits Resources. Any money in your account after this deadline will be forfeited with one exception: If you participated in a 2015 Health Care FSA last year, you can let up to $500 carry over into 2016.
  • Don’t forget to sign up for a Take Action activity! Simply log on to staywell.com and choose an activity on the To-Do list. Complete up to two activities to earn a credit toward your 2017 medical premiums when you also take the Health Risk Questionnaire (HRQ) this fall.
