  • Our 2016 retirement webinar series starts this month.   • Check your payroll deductions. It’s a good idea to review the health care deductions on your first paycheck in January. If something is inaccurate, call the Rockwell Automation Service Center at 877.OUR.RASC (1.877.687.7272).
  • Watch for your 2015 tax forms. Your 2015 W-2 will be available later this month. If you requested an electronic version, it will be on EmployeeConnect (under the “Your Pay” tab) by Jan 31. Otherwise, your W-2 will be mailed to your home address on Jan. 31 with delivery by Feb. 15. The other form you need this year, Form 1095-C, will be ready by Feb. 1, and will be delivered separately from your other tax forms.
  • If you had a Health Savings Account last year, remember that you need to include Form 8889 with your 2015 federal income tax return. 
  • Fidelity will mail 2015 year-end Retirement Savings Plan statements to your home address later this month. You can also request online statements at any time by visiting 401k.com and logging in to your account.
  • Get access to UnitedHealthcare’s tools and resources! If you’re enrolled in a Rockwell Automation medical option, be sure to register for the myuhc.com, download the Health4Me app (for Google and Apple products) and save the UHC number (844.234.7924) to your contacts. t includes access to the 24/7 nurse line. These three resources are your gateway to all the information, tools and resources UHC has to offer. Start exploring them today.
  • Enrolled in vision only? If you’re enrolled in the vision plan, but not a UHC medical plan, you can register on UHC’s vision portal at myuhcvision.com. To register, use the policy/group number 904936, last four digits of your Social Security number and date of birth as identifying information. Then enter your contact information and choose your user name, password and PIN. You can use UHC’s main number (1.844.234.7924) for vision questions or call the vision provider locator number for help finding local providers (1.800.839.3242). Participants enrolled in vision and medical can reach the vision portal from myuhc.com.
  • Take the Health Care Provider form to your annual physical. As your provider captures your weight, blood glucose, cholesterol and blood pressure, document it on the Health Care Provider form. Then submit the form to StayWell by Dec. 31 to complete your health screening and earn up to $250 off your 2017 medical premiums when you also take the Health Risk Questionnaire in the fall.
