  • Last chance to earn up to $450 off your 2015 medical premiums. If you’re participating in the Health Management Program, be sure to get your health screening, complete the Health Risk Questionnaire (HRQ) and/or finish your Take Action activity by Dec. 31.
  • In most cases, you’ll continue to use your current medical/prescription drug ID card in 2015. You’ll receive new ID cards in December only if you’ve switched medical plans, changed your covered dependents or changed your address. If you need a dental or vision ID card, you can print it from the MetLife website.
  • The deadline to incur expenses for your FSA is Dec. 31. If you have a Health Care FSA, up to $500 can roll over to 2015. Any other FSA funds not used by the end of the plan year will be forfeited. Keep in mind that March 31, 2015, is the deadline to submit your claims. In most cases, the only document you need to substantiate your Health Care FSA claim is the medical or dental EOB (Explanation of Benefits).   • To receive your W-2 electronically, visit EmployeeConnect no later than Jan. 15. Click the “Your Pay” tab and go to “Taxes and Deductions” to consent to the electronic version. (If you have previously registered, you do not need to register again.)
