UnitedHealthcare Medical and Vision ID Cards

Your ID card has key information about you and your coverage. When you visit your doctor, hospital or other health care provider, remember to show them the card so they know how to bill for the services they’re providing you. If you completed your medical enrollment by Nov. 15, 2015, you will receive an ID card by Dec. 30, 2015. If you need urgent medical care on Jan. 1 and don’t yet have your medical ID card, get the treatment you need and provide your insurance information later.

Who Gets an ID Card

When you have coverage for yourself only, you will receive one ID card. When you cover family members, you will receive one ID card for yourself and one additional card for your family, no matter how many dependents you cover. All covered dependents will be listed on both cards.

Requesting an Additional Medical ID Card

If you don’t receive your UHC medical plan ID card this month or you need an additional copy for a family member, you can print a card online or use the mobile app to view, fax or email your ID card. First, register for myuhc.com on or after Jan. 1. There are two ways to register: you can register using the member number on your card and the Rockwell Automation group number: 904936; if you don’t have your ID card, you can register using your Social Security number and date of birth. (You only need to use your Social Security number if you don’t have your ID card.)

  • To print a card or request a replacement, click on “Account Settings” and then “Temporary Card.”
  • To view your ID card on your phone, download the free Health4Me app through the iPhone and Android app stores. Then use your myuhc.com username and password to get started.

When You See a Provider

Bring your ID card and a government-issued picture ID, such as a driver’s license. Show them when you check in at the doctor’s office.

Call the member phone number on your ID card (the same number provided below) to learn if any restrictions apply and to find network providers near you.

ID Cards for the Vision Option

UHC vision providers don’t require ID cards because providers can verify your coverage with your name and date of birth. However, if you or a family member would prefer to have one, you can print it from a link on the vision portal home page or view it on your smartphone. There are two ways to get to the vision portal:

  • If you enrolled in UHC medical and vision coverage, after you register for myuhc.com, log in anytime starting Jan. 1. From the “Look Up My Benefits” link, select the link for Vision Benefits.
  • If you enrolled in vision only, go to myuhcvision.com and register using the last four digits of your Social Security number and your date of birth.
